1 It is a stand-alone Care Standards Monitoring & Review Team. The Review Team which covers Older Persons, Learning and Physical Disability annual reviews are part of the team.  Reviewing Officers are trained to assess standards of care delivery and audit provider paperwork as an example MAR charts for the service user they are reviewing.  If they highlight any concerns they will then report them on the E Form described below and the concerns will be actioned by the Care Standards Monitoring Officers.

2  We have an E Form reporting system to enable front line workers who carry out assessment and reviews, to report standards of care delivery either good or bad evidenced by the worker or reported by service user or family.  This information forms part of the intelligence used by the Care Standards Monitoring Team to decide on the level of input required.  We would then request evidence, carry out a site visit or initiate a full assessment dependant on the seriousness of the report.

4 Customer feedback regarding their provider is reported by front line workers on behalf of the service user via the E Form.  Formal Complaints received relating directly to a provider are usually addressed by the Head of Care Standards & Review and follow the corporate complaints process.   Surveys are conducted by the Commissioning Team.

5 When carrying out Monitoring Assessments whether for external or internal provider services we audit for example MARS Charts, Care Plans, Risk  Assessments for both internal and external provider services. 

6 Yes, members of the team are involved in some service specs and assessing and scoring some tender specifications.

7 The team are involved in most strategic decisions relating to provider services but is not involved in the Commissioning/Contracting of services.  The Commissioning Team is a separate Team.

1 How many days per year do you make available ( either as a direct service or a commissioned service) holiday play scheme provision for children resident in your authority that have a disability.
In terms of that disability I am referring to children ( up to 18) that have a Statement / EHCP.
I would like this information for each classification of disability ( i.e Complex needs, SLD, Physical disability etc – however you choose to classify) and based on the current year provision and any planned changes to that for the next financial year. If you can provide per holiday period i.e how many days of provision per half terms, how many days at Easter, Christmas and Summer.
This is not a question of what each family has access to (clearly that is case to case)  but rather what is made available by your Authority for the individual family to request/apply for.
2 It would also be helpful to know the length of those days offered on a play scheme- i.e is it 10-3 or 9-5 or is there some flexibility to suit parents/carers such as the option to extend by an hour or do so called ‘early bird’ drop off??

Following public consultation Bedford Borough Council determined at a meeting on 16 January 2013 that it would apply the 50% premium to all long term empty dwellings from 1 April 2013.   I can, therefore, confirm that Broad Avenue has been subject to a Long Term Empty Premium of 50% between 1 April 2013 and 10 October 2014, which is payable in addition to the ongoing Council Tax liability

Under Section 12(5)(e), which relates to the confidentiality of commercial or industrial information where such confidentiality is provided by law to protect a legitimate economic interest, we are not required to provide this information.  This is because the list of footway and carriageway works for the financial year 2014 2015 is available from the Borough Councils website, the link is here.  Both last year’s works and this years are available to view.

With respect to forecasted and final outturn costs we deem this to be sensitive information as this could be used with the rates to determine the contractor’s submitted rates which is commercially sensitive information.

Thank you for your request for information about Dyke Rd Brighton received on 9th April 2015. Your request has been considered under the Environmental Information Regulations 2004. I have answered your query below:

By virtue of Part IIA of the Environmental Protection Act 1990 the Council was required to formulate a Strategy to detail how it plans to meet its duty to inspect land in its area and identify contaminated land under sections 78B and C of the Act. The Works site is included on our prioritised list of potentially contaminated sites, and will be assessed as part of our inspection arrangements. This site has been given a medium priority for inspection. It is not possible to state when this assessment will be undertaken. At this stage, the Council is unable to state whether or not it expects this site to be identified as contaminated land because its inspection of land is in the course of completion. Therefore no reply given above refers to contaminated land for the purposes of the said Act. Similarly, unless otherwise noted, references to contamination should not be taken to mean the land has been deemed prejudicial to health or a nuisance for the purposes of Part III of the Act.

we are looking to purchase in Harrold and the fact we are trying to find proof that the petrol tanks have been removed (clause 12).

Thank you for your request for information about Eagle Way, Harrold, which we received on14th April 2015. Your request has been considered under the Environmental Information Regulations 2004. I have answered your query below:

The Petroleum Officer, Richard Chattaway, confirms that he believes that the site was formerly occupied by Eagle Works, Harrold an old tannery. Our records indicate that the tanks were used for diesel from 1985 onwards and that they were all cleaned. There would be no records after that, as there would have been no requirement to visit.
