Support planning and brokerage

Service Description

If you have a personal budget as a direct payment, individual budget or individual service fund we can support you to plan your service and help you to make decisions about how you will spend your budget, what you will spend it on, and who you will spend it with.

Using person centred planning tools and techniques, we will work with you to explore the support options available to you, whether paid or unpaid, and to plan and organise the support you need to keep you safe, fit, well, active and happy.

Depending upon the level of support planning you are looking for, we can:

  • Help you to map out services you’d like to engage with to support you to lead a full and active life
  • Support you to find ways of maximising your budget and benefits by exploring unpaid support networks, activities and local opportunities
  • Work with you to create and write your own personalised support plan with associated costs
  • Liaise with your funding authority on your behalf to agree your support plan and personal budget
  • Work with you to find a provider to deliver your service, including arranging and supporting meetings
  • Support you to recruit a personal assistant, from writing a job description and sorting applications to interviewing
  • Attend your review to make sure that what you wanted is actually in place and working for you
