Consultation documents

We know filling in forms can be boring, but:

These two consultation booklets have been designed for children and young people of different ages and abilities to make sure your views are recorded and heard at your looked after children review;

click on the images below to take a look and see which one you would prefer to fill in.

Your Review booklet cover

Your review over 10

This booklet is about you….

  • what you like
  • what you don't like
  • how you feel about where you live
  • your education
  • those close to you

Filling in a booklet would help everyone at your review to talk about what is important to you.   It is really important we know your wishes and feelings.

This is to make sure we are listening to you and making the right decisions with you about your life.

Your Independent Reviewing Officer (IRO) will talk through with you the information in your booklet before your review.

Don’t forget, you need to be at your review to tell people what you want and what changes we can make for you.

Consultation Booklets for Parents & Carers

It is really important that you make a meaningful contribution at the review so that an informed decision can be made about the short and long term needs forming your child / young person's Care Plan.

As the parent/s or carer/s for a child/young person who is being Looked After, it is essential that you contribute to the review meeting.

The meeting is chaired by an Independent Reviewing Officer, who is independent from the social worker.  They are responsible for ensuring that the Local Authority has the right Care Plan for your child / young person in order for them to achieve their full potential in their care.

What you need to do to support your Child / Young Person at their Review

  • Attend all reviews when invited;
  • Think carefully how you can offer ongoing support to the young person;
  • Complete the Consultation Booklet prior to the meeting so the Independent Reviewing Officer can understand your views, feelings and the support you can offer
  • Inform the review of the achievements of the young person, their ongoing needs, wishes and feelings from your perspective.

The Independent Reviewing Officer

  • Wants your views;
  • Will listen to your views;
  • Will ensure that your child/young person’s needs will be met in order to ensure that their developmental needs are a priority to the Local Authority

The Independent Reviewing Officer is contactable (see Independent Reviewing Officer) to you in addition to your completion of the Consultation Booklet
